Race Pack Collection Power of Attorney
Use this authorization form if you are unable to collect your Race Pack yourself and need to delegate it to someone else. This form must be completed to collect the race pack on your behalf, and must include the following supporting documents:
1. Printed registration confirmation email
2. Copy of the participant's ID card
